Integrated Care Clinic

The Integrated Care Clinic at St. Paul's Hospital provides care for patients with kidney disease who also have diabetes and/or heart disease. The clinic brings together medical specialists (nephrologist, cardiologist and endocrinologist), as well as a knowledgeable team of nurses, pharmacists, dieticians and social workers.

The goals of the Integrated Care Clinic are to improve communication and coordination of patient care, to reduce medical appointments and duplicate testing for patients, and to provide more integrated education and self-management for the patient. Patients are referred to the Integrated Care Clinic by a family doctor or specialist.
