Providence Building 1081 Burrard Street Vancouver, BC V6Z 1Y6 Phone: (604) 806-9016 Fax: (604) 806-8076 The Post-Transplant Clinic monitors, teaches and supports patients and their families for the entire life of their new kidney. Immediately after discharge from hospital, our team will see patients up to twice a week, a schedule that will eventually decrease to once every 3 or 4 months. Post transplant services include: follow-up care and education for all renal transplant patients, monitoring patients by lab work, clinic visits and telephone, and facilitating transfers to and from regional transplant clinics and other transplant centres.
